Elissa Hamilton,RN, PHN, NA-BC, MBA

Elissa Hamilton, Director

Elissa is a registered nurse and has more than 30 years of Home Health management experience. With a Bachelor of Science Degree in Nursing and a Masters in Business Administration, she is also Board Certified in Nursing Administration by the American Nurses Credentialing Center and is active with the California Association for Health Services at Home (CAHSAH). Elissa is a former Board member, currently sits on the Medicare Committee and was selected to fill an elected seat on the CAHSAH Nominating Committee. She is a fervent advocate for home care and feels that any one who wants to remain in their home should have the option to do so.

Tara Pardo

Home Care Manager
Tara Pardo, Supportive Home Care Manager

Tara Pardo, a Home Care staff member since 1993, is the current Home Care Manager for Private Home Care and Palomar Health Home Caregivers. In 2004, Tara designed and initiated the Supportive Home Care Service line at Palomar Health and recently added the new Care Management model to the district’s services so that outpatients can continue to receive comprehensive health care.

Tara serves on the California Association for Health Services at Home (CAHSAH) licensed and nominating committees. She is active at Palomar Health and in the community advocating for access to care and services. Her involvement includes participating in the Palomar Health Community Action Council, Service Excellence Team, and San Diego Caregiver Coalition.

Elisa Cullaty, MSW

Care Manager

Elisa Cullaty, MSW has a Masters Degree in Social Work from Boston College and a Masters Degree in Psychology from Harvard University Extension School. She brings over 20 years of counseling and social work experience to her new role. Her past work in home care and hospice has given her a strong foundation in outpatient services and community resources. She has also worked as a Care Coordinator at Pomerado Hospital in the Behavioral Medicine Unit. Her role includes home safety evaluations, Care Management, and referrals to community resources.
