Available 24 hours a day, 7 days a week, MyPalomarHealth provides free, secure online access to a portion of your medical records. View your health summary, review test results, and access trusted health information.

MyPalomarHealth is available upon registration at any Palomar Health hospital. At registration, you will have the option to receive an email invitation with instructions on activating your account.

Setting Up a New Account

If you are an existing patient of Palomar Health and will be visiting one of our hospitals soon, you will be provided the opportunity to sign up for MyPalomarHealth through the registration process during your admission. At registration, you will have the option to receive an email invitation with instructions on activating your MyPalomarHealth account.

We take great care to ensure your health information is kept private and secure therefore, in order to start the process, you will be required to visit the Medical Records Department at Palomar Health Downtown Campus to submit the forms  along with proper identification. If you would like to request access to MyPalomarHealth, please complete the appropriate form(s) below:

  1. Patient Portal Consent Form
    This form allows a patient or representative access to MyPalomarHealth, a website to view their patient's health information.
  2. Patient Portal Proxy Authorization Form
    Please complete this form if you are an adult patient at least 18 years of age and want to give another adult, of your choosing, access to your patient portal account.
  3. Patient Portal Proxy Authorization Form (initiated by someone other than the patient)
    Please complete this form if you are an adult at least 18 years of age, are legal guardian or have durable power of attorney for healthcare, of an adult patient and you are requesting proxy access to that patient’s portal documentation.


Revoke Access to MyPalomarHealth

If you no longer wish to participate in MyPalomarHealth or have access to MyPalomarHealth, please fill out the Revocation of Authorization Form and submit it to the Medical Records Department at Palomar Health Downtown Campus.


Make a Change to Your Medical Record

A Medical Record Correction/Amendment Form must be completed and submitted in person with your identification to the Medical Records Department at Palomar Health Downtown Campus. The form can be accessed online at the following location on PalomarHealth.org: www.palomarhealth.org/ContentPage.aspx?nd=965 by selecting the Medical Record Correction/Amendment link.
