Palomar Home Health Services

Palomar Home Health Services is a Medicare Certified Home Health Agency that has been servicing San Diego North County and Southern Riverside Communities since 1985. The agency specializes in a variety of health care services for adults and infants, provided by a staff of nurses, physical therapists, occupational therapists, speech pathologists, social workers, and certified home health aides, who are all trained to provide the highest level of care and customize our services to meet the patient’s needs based on their diagnosis, physician orders, and Medicare, MediCal and other insurance coverage requirements.   
 

Telehealth Monitoring 

Telehealth Monitoring allows the nursing staff to closely monitor high risk congestive heart failure (CHF) patients and work with the patient’s physician to help keep the patient safely at home.


Private Duty Care

Private Duty Care through Palomar Health Home Caregivers provides clients with personal care attendants, who assist the client with all of their non-medical home care needs. The care is customized based on the client’s needs, whether they require 4 or 24 hours of care a day. The services also includes New Mother Assistance and a special program called Rides with Care  which provides patient transportation home from the hospital or Skilled Nursing Facility, or just to go from their home to an appointment.  Since Palomar Home Health Services is a licensed home health program, the agency can also provide shift care with LVNs and Certified Home Health Aides under long term care insurance plans.
 

Maternal Infant Program 

The Maternal Infant Program provides specialized services in lactation education and support by RN lactation educators and RN lactation consultants.  Services also specialize in early hospital discharge follow-up, assessment of infant weight loss, feeding difficulties, and jaundice; high risk post-partum visits, and post-partum depression screening and referral.  Nurses provide education in newborn care and home safety, maternal/infant nutrition, Sudden Infant Death Syndrome (SIDS) precautions, and prevention of complications leading to readmission to the hospital.  Nurses provide referrals as needed to San Diego First 5 grant programs which provide community-based home visiting services and parenting classes, public health nursing, child welfare services, and Nutrition Program for Women, Infants, and Children (WIC).  Nurse visits also provide in-home monthly injections of Synagis prophylaxis for the prevention and control of Respiratory Syncytial Virus (RSV) to medically fragile infants. 
 

Community-Based Care Transitions Program (CCTP)

Palomar Home Health Services has been actively involved in the Community-Based Care Transitions Program (CCTP), a unique demonstration grant in which Palomar Health, Scripps Health, Sharp, UCSD hospitals and the County Aging and Independence Services collaborated to design an innovative program for high risk Medicare patients in our communities.  The San Diego collaborative became a model for the nation and while grant funding has been discontinued, Palomar Health has continued the important process redesigns that led to the success of the program in supporting the needs of our high risk Medicare population.
